Together with you and your family, the Case Manager Nurse will plan and personalise your treatment pathway during your stay in the clinic, along with other members of the health team: doctors, nurses, physiotherapists and dieticians.
Planning and assessing your treatment pathway will involve:
- The first meeting with the Case Manager Nurse will take place when you are invited to the pre-admission appointment. After the blood tests, the ECG and the appointment with the anaesthetist and unit doctor, you will speak with the Case Manager Nurse. They will explain which treatment goals they will aim to achieve before you are discharged and sent home. In this regard, they will also assess whether a home care service needs to be planned and arranged, in order to assist you and your family in your gradual return to health.
- A second meeting will take place two days after the surgery, including an initial assessment of the planned treatment goals.
- Before you are discharged, there will be a third meeting where a final assessment will be made to prepare for your return home or, where necessary, to a rehabilitation facility.
Your treatment pathway doesn’t end when you are discharged: after 30 days and then after one year of returning home, the Case Manager Nurse will contact you by phone in order to enquire of your state of health and answer any questions you may have.
